Welcome! If you are new to the area or would simply like information about enrolling your kindergartener, you’ve come to the right place. Here you will find general information about our enrollment policies (including immunization requirements) and can even begin the process by downloading the forms. Registration

If you are interested in joining our Lukfata Falcon family, we encourage you to make an appointment for a school tour. Tours are a great way to acclimate incoming students, and they give you a better idea of the excellent programs and opportunities we can offer your family!

Once your initial paperwork is complete, please make an appointment with our school office, and bring:

• Your child’s original birth certificate
• Updated immunization records
• Proof of residency (see district map)

If your child is transferring from a different school, please include his/her school records or a completed Request for Records form authorizing the Lukfata school district to obtain the records for you.

Immunizations

Oklahoma law requires children to receive certain immunizations in order to attend public schools. These requirements are for the health and safety of all students. State law also dictates that updated immunization records be kept on file as part of each student’s permanent record. Should a disease outbreak occur, the information on these records is used to ensure our students are protected.
